The COVID-19 pandemic is one of the greatest challenges facing humanity, it has put unprecedented pressure on health systems around the world and health care is experiencing the greatest patient safety crisis since it has put highlights the difficulties that health workers face. Analyze the situation of the patient safety culture and describe the initiatives implemented to improve it. A narrative review was carried out, the search for articles was carried out in indexed journals, research from the last six years was considered. The safety culture was considered weak in 31 articles and of the 12 dimensions of the patient safety culture, only 4 dimensions reached scores greater than 75%, the health personnel described not having reported any adverse event, for fear of punishment. To improve patient safety culture, organizations must be able to quickly detect and address problems and factors that contribute to system failures. One of the most effective ways to identify these factors is by reviewing and investigating errors reported by staff.
